3. 遞交申請表時，請附下列證明文件之影印副本。
   Please enclose a copy of the following documents:
   
   
   1. 學生的香港身份證／護照
      Student’s H.K. ID Card / Passport
      
    
   2. 中學學位分配證或小六學生證
      Secondary School Places Allocation Form (SSPA) or Student’s Primary Six Identification Card
      
    
   3. 學生最近兩年的成績表
      School reports of the last two years
      
    
   4. 其他附加資料（如證書或獎狀等，尺寸不超出A4）
      Other relevant documents (certificates and awards not bigger than A4 size)
      

4. 遞交申請表請連同HK$50報名費之劃線支票（報名費恕不退回，抬頭請書「港大同學會書院」），支票背面須寫上申請人姓名。
   A non-refundable application fee of HK$50 by crossed cheque (made payable to “HKUGA College”) with the applicant’s name at the back of the cheque should be submitted with this application form.
   
   
5. 附上3個分別貼上足資郵票及寫上申請人姓名地址之回郵信封。
   Enclose 3 self-addressed and stamped envelopes with the applicant’s name and address.
   
   
6. 申請人將獲邀參與2012年1月7日進行之小組面試。合資格者將獲邀參與次輪面試，時間及地點將另行通知。
   Applicants will be invited for a group interview on 7th January, 2012. Only short-listed applicants will be invited for a second interview. The time and place for the interview will be announced later.
   
   
7. 獲得取錄之申請人將於2012年3月底前接獲本校書面通知。
   The School will notify successful applicants in writing by the end of March, 2012.
   
   
8. 本年度(2011-2012)中一全年學費為HK$35,000。
   The annual school fee for the current year (2011-2012) is HK$35,000.
   
   
9. 根據 «個人資料(私隱)條例 »，你有權查閱或更改所提供的個人資料。
   According to the Personal Data (Privacy) Ordinance, you have the right to access and amend the personal data provided on the application form.
   
   
10. 如須查閱或更改個人資料，請以書面提出並寄往港大同學會書院。
    Requests for access to and amendment of personal data should be made in writing and sent to HKUGA College.
    
    

The College has put into place a “Fee Remission Scheme” for students in need of financial assistance. “Scholarships” will also be offered to students who have demonstrated excellent academic performances, or outstanding achievements in art, music, or sport.
